* feat(o11y): conform telemetry labels to the five-label identity set

Replace the site label with provider and region, completing the
project/env/cluster/provider/region contract from o11y's shipping guide.
father ships provider=hetzner region=fsn, luke ships provider=int
region=aus (the region.hcl codes). Nothing queries site, so the rename
drops nothing; changing the label set re-keys the shipped series, which
costs one retention window of duplicate index on the o11y stores and
false-fires nothing (cluster and env are unchanged).
